Inside Wire Maintenance - Terms & Agreement

As a Supra Telecom customer, you are responsible for maintaining that part of your telephone line between the Network Interface Device point (Which is usually a gray box attached to the outside of your house) up to and including your telephone set. To help you maintain the jacks and wiring in this part of your line, Supra Telecom offers you an optional Inside Wire Maintenance Service Plan. The Plan is subject to the following terms:

1. You agree to pay a monthly charge for the plan. There is no additional charge for repairing or isolating problems in the wiring or jacks inside your home or business.

2. We will locate the source of and repair telephone service problems which may exist in the wiring and jacks inside your home or business. If the problem is caused by a defect in the cord from the jack to the phone or other equipment (e.g. fax machine, answering machine, modem, etc.) that is attached to your line, we will advise you which phone or other equipment is the source of the problem. We do not repair such defective phone cords, phones, or equipment.

3. If you have more than one access line in your premise, you must subscribe to the Plan for each such access line.

4. You are not eligible for this plan if you have a key system, a private branch exchange (PBX), or other non-basic telephone system.

5. SUPRA TELECOM SHALL NOT BE LIABLE FOR DAMAGES, INCLUDING ANY INDIRECT, INCIDENTAL, OR CONSEQUENTIAL DAMAGES THAT ARISE FROM:

a. Any defect in materials used to maintain inside wiring or jacks; or
b. Defects in workmanship provided under plan. There are no express or implied warranties.

6. This plan is optional. Non-Payment of charges for this plan will not cause termination or denial of your regular telephone service. However, non-payment of plan could result in cancellation of the plan and the application of late payment charge. Any late payment charge will be at the same rate imposed by Supra Telecom for other late payments charges as set forth in our applicable tariffs.

7. This plan does not cover the following:

a. Problems caused by willful damage to inside wire or jacks
b. Damage caused by Acts of Nature (such as flood, hurricane, fire windstorm or similar acts)
c. Service problems in your inside wire or jacks that were in existence at the time you subscribed to the plan.
d. Inside wire or jacks that do not meet industry standards for telecommunications.
e. Repair to and/or maintenance of your inside wiring or jacks to the extent that after reasonable effort we determine that such repair/maintenance cannot be performed in a safe manner due to the presence of asbestos or any other environmentally hazardous substance or due to the existence of any other unsafe condition.
f. Where existing customer request coverage under the plan, the plan will not become effective until 30 days after the date in which the plan is ordered.This plan is provided on a month-to-month basis and can be cancelled by either party giving oral or written notice to the other.

8. Once we determine the source of your telephone service problem and if that source is not covered under this plan you will not be charged for the technicians visit or diagnosis of the problem. However, if you opt to have the technician proceed in repairing or replacing the source which is not covered under this plan you agree to pay the current applicable installation and/or repair rates.

9. This rate and other terms or conditions may change from time to time with 30 days notice. Such notice may be provided in a bill insert, as a message printed on our bill, in a separate mailing, or by any other reasonable method at our discretion. This shall include the revised rate, terms or conditions and the effective date of the change. If the rate, term or conditions are changed and you do not wish to continue as a subscriber to this plan, you may cancel by calling your local Supra Telecom office. Unless you cancel this plan the revised rate, terms and condition will automatically apply to you.
